Summary:
Provides skilled nursing care ordered by the physician as indicated by the plan of care to all patients on caseloadReview caseload with supervising RN on a regular basis as established by best practice and/or State guidelinesObtains necessary physician orders and collaborates with the physician as the patient's needs dictateEvaluates the significance of assessment findings and communicates pertinent information about the patient's clinical status and ongoing needs to others in a timely fashionGuarantees that changes in patients' clinical status are communicated to other healthcare team members, necessary office staff, and physician as per Agency policyEducates patients, caregivers, families, and other staff as appropriatePerforms aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errorsParticipates in case conferences to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etcRecommends for needed referrals to other disciplines and/or community resourcesFollows the seven medication rights and reduces the potential for medication errorsFollows up with obtaining lab results and notifies the physician of significant changesParticipates in the agency Quality Assurance Performance Improvement (QAPI) programIdentifies risk of injuries or threats to life and health in the patient's environment (e.genvironmental/safety/mobility risks) and implements appropriate interventions to reduce those risksCompletes accurate and thorough visit notes via Electronic Health Record (EHR) on every patient in a timely fashionEducation/Experience Requirements:
Graduate of an accredited nursing programCurrent licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) in state of practiceExperience as an LPN/LVN commensurate with one of the following preferred:
One year experience in an acute care setting within the last 24 months; Six months home care experience within the last 12 months preferredValid driver's license and reliable transportation Proof of automobile insurance (MVR checked) Proof of current CPR certification Able to pass criminal background check and national sex offender clearance Career

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
