The Ryan White Care Coordinator will provide non-emergent medical transportation rides for consumers through a voucher system or ride share, to medical, dental and vision appointments, pharmacy medication pick-ups, lab work, and other related servicesThe coordinator will assist with collecting trip information, trip logs, creating PCF for delivery of services, maintaining spreadsheets, and assisting with high call volumeThe coordinator will assist with all voucher programs, PCF approval process of payments and process consumer NMAP referral to enroll and determine a payer source for servicesThe coordinator will act as a representative of Access to Healthcare Network (AHN) while on duty and will relay information about other programs to consumers as well as problem solve on behalf of our consumers to ensure that they receive a continuum of care and supportKey responsibilities and accountabilities:
Gain and maintain a general working knowledge of Ryan White and HOPWA program servicesGain a clear and working knowledge of the HIV/AIDS Drug Assistance Program (ADAP); establish and maintain appropriate working relationships with the persons and agencies involvedGain clear and working knowledge of granting NMAP (ADAP) dates based on payer of last resource rule (30 days vs full NMAP dates)Learn, and follow, the payer of last resort rule at all timesScreen eligibility applications received from Part A and Part B agencies to verify the application is completed correctly and all required documents are attached for eligibility and ADAP servicesManage all incoming eligible Ryan White referral data entry from Part A and Part B CAREWare systems in a timely mannerFor Part A& B Referrals ability to transfer (mirror) incoming referral data into Part A, Part B and SalesforceGain clear and working knowledge of social services available throughout our community; establish and maintain appropriate working relationships with persons and agencies involvedGain/Possess a clear and working knowledge of Salesforce, Mitel and CAREWareComplete an eligibility screening to ensure consumers are eligible for assistance based on funding sourceGather information from AHN Ryan White consumers to determine the payer of last resort for Medical Transportation ServicesRefer eligible consumer to AHN Transportation departmentActs as a liaison between consumers to identify other AHN/Community programs of which consumer may be eligible forAccurately and efficiently record all pertinent consumer information into CAREWare and SalesforceEntering, transcribing, recording, storing, or maintaining information in written or electronic formReturn all phone calls, emails, or written correspondence within 48 hours of receiptProcess medical transportation referrals for consumers within 48 hours of receipt unless otherwise noted by a provider and in accordance with AHN policies and proceduresOrientate Program members with specific program informationTrack program member appointments and compliance with program guidelines Track funding and process payments for ride shareGain a clear knowledge of PCF approval processComplete PCF approval weekly to ensure payments are processed timelySubmit all programmatic reports on time, in a professional format and with accurate dataComplete daily input of all services provided to consumersBe able to multitask and critically think though a variety of situationsPerform other duties as assigned
Qualifications:
Minimum Education:
High School Diploma Minimum one year computer experience including Microsoft operating systems, particularly Excel, Word, and OutlookDatabase input experience and the ability to use a smartphone requiredRequired to pass a background check through the State of NevadaSubject to Medicare/ Medicaid fraud background check Required to pass a drug test upon hire and may be subject to random drug testing throughout employmentBilingual preferred (English/Spanish)Extreme patienceIs consistently at work and on time and arrives to appointments and meetings on timeBe able to manage one's own time and the time of others efficientlyBe able to actively learnBe able to operate at the pace of business to meet deadlines, plan and prioritize, and remain calm when dealing with difficult customersBe able to adapt to changes, delays, or unexpected events in the work environmentAbide by all HIPAA requirementsMust attend staff meetings and be willing to meet with supervisor on a consistent and regular basisGain a clear and basic knowledge of Access to Healthcare Network Services Ability to be both compassionate and professional while serving our populationAbility to adapt to changing work environment and dutiesOther duties as assignedExcellent customer service skills and responds promptly to customer needs in a respectful mannerAble to manage difficult or emotional situationsStrong, professional verbal and written communication skillsBe able to bend, walk, sit, squat, reach, twist, push, pull, grasp, kneel, crawl, and/or climb with a fair consistencySpecific vision abilities include close vision, distance vision, color vision, peripheral vision, and the ability to adjust and focusBe able to withstand hand/wrist deviation and repetitionExcellent communication and customer service skills Be able to stand between 1 - 2 hours per dayBe able to sit in a vehicle or at a computer for up to 6 - 8 hours per dayMust be able to lift, carry up to 30 pounds with a fair consistencywork environment:
