The Clinical Social Worker is a critical position within the Sierra Nevada Health Care System (SNHCS). Under the direction of the HCHV Program Supervisor, this case manager will coordinate and provide services supporting the functional abilities of veterans who are struggling with issues of homelessness. The clinical social worker will work directly with veterans within various community housing settings and community sites and will provide supportive case management services and partner with community agencies to link access to community-wide housing services for veterans. This social worker will be providing services such as psychosocial assessments, treatment planning, individual, group therapy and follow-up, crisis intervention, case management assistance, resource development, and coordination related to these services as clinically indicated. This incumbent will provide case management and referral assistance to the veterans and work closely with the primary care clinicians and the interdisciplinary teams that assist consumers in meeting their goals and objectives within a psychosocial rehabilitation framework that allows successful reintegration into community living.
The majority of veterans requiring the incumbent''s assistance require supportive treatment provided through a well defined program which includes psychiatric assessment, supportive therapeutic supervision, clinical case management, medical consultation, bio-psychosocial treatment and rehabilitation, supportive group and or milieu therapy, psycho-education services, occasional vocational counseling and rehabilitation, substance abuse counseling, and socialization within an outreach treatment format. The program is comprehensive and requires extensive coordination among a wide variety of staff and treatment components to ensure its success. The program requires active interface with the staff of other outpatient and inpatient treatment components as well as networking with community agencies and Medical Center Staff.
The incumbent is responsible for the development and coordination of emergent and temporary housing opportunities and services and to ensure that these components are commensurate with the goals and objectives of the medical center and its recovery model of care. The incumbent is required to reassess in an ongoing manner the changing and evolving needs of the veteran population.

The incumbent will interface with HCHV Program Supervisor on a regular basis related to matters of service delivery, program development, and job definition. The incumbent will also interface with colleagues from other VA facilities in order to ensure efficiently coordinated services to those members of the Veterans Integrated Service Network, as well as the local community, including housing authorities, shelters, and landlords.
The incumbent, as a clinical case manager is responsible for the ongoing process of review, development, treatment planning, and interventions with veterans as assigned.
The nature of the work will encompass a wide range of Psychological, social, and environmental issues. Incumbent is therefore expected to possess and employ an effective range of social work knowledge and skill in providing effective intervention and resources.
Effective performance will require a good knowledge of community resources and various treatment modalities. Incumbent is expected to participate in clinical staff meetings, rounds, and program planning meetings within their assigned area as appropriate to the effective execution of their assignment.
Responsibilities may vary according to unit and service assignments, and job level. Incumbent may be required to perform case management functions within client''s home community, and these functions may include transportation of patients.
Responsibilities may also require training and participation in dealing with violent and disruptive patients.
Incumbent is expected to assume responsibility for all cases in their assigned area requiring social work services, and at times be required to provide appropriate clinical backup and guidance to social work and other unit staff.
As a member of the social work discipline, the incumbent will be expected to attend service and unit meetings, and to serve on committees and task forces related to the effective operation and Quality Assurance of the service.
Incumbent is also expected to maintain ongoing professional development to assure clinical knowledge and skill levels pertinent to the adequate performance of their duties and Q.A.
The incumbent will comply with agency directives, circulars, procedures and policies; maintain records and statistics; prepare correspondence; make entries in the clinical record; and carry out special projects as required.
Provides consultation and assistance to HCHV program including NEPEC data reporting as required.
Provides social work consultation to other professional staff both inpatient and outpatient. He/she may be called upon to provide supervision to graduate level Social Work Interns, and other students.
Upon occasion, when requested, the incumbent may assume the responsibilities of the Director, Homeless Program in his/her absence.
The incumbent is professionally and administratively responsible to the HCHV Program Supervisor. He/ she are expected to seek consultation when needed and keep the program management informed of significant developments.
The incumbent has latitude for determining day to day work activities and decisions, and is expected to make independent judgments and decisions in providing services to veterans, regardless of complexity and difficulty of the problem presented.
Incumbent is also expected to attend weekly clinical and administrative supervision.

Consistently communicates and treats customers (veterans, their representatives, visitors, and all VA staff) in a courteous, tactful, and respectful manner. Provides the customer with consistent information according to established policies and procedures. Handles conflict and problems in dealing with the customer constructively and appropriately.
Protects printed and electronic files containing sensitive data in accordance with the provisions of the Privacy Act of 1974 and other applicable laws, Federal regulations, VA statutes and policy and VHA policy. Protects the data from unauthorized release or from loss, alteration, or unauthorized deletion. Follows applicable regulations and instructions regarding access to computerized files, release of access codes, etc. as set out in the computer access agreement that the employee signs.
The staff member must be competent to demonstrate the knowledge and skills necessary to provide care/services appropriate to the age of the patients served on their assigned unit. Individuals must demonstrate the knowledge and ability to assess data reflective of the patient''s status and interpret the information needed to identify each patient''s requirements relative to their age-specific needs and to provide care needed as described in the unit''s/area''s/department''s policies and procedures.
BASIC REQUIREMENTS:
Citizenship: Citizen of the United States (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy)
Education: Must possess a Master''s Degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the Master''s Degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a Master''s in Social Work Degree (MSW)
Licensure: Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the Master's Degree level. Current state requirements may be found on the OHRM website
Physical Standards: Applicants must meet physical standards for the position. A physical examination prior to placement is required. See VA Directive and Handbook 5019
English Language Proficiency: Individuals appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d)
Creditable
Experience: Knowledge of Current Professional Social Work Practices. To be creditable, the experience must have required the use of knowledge, skills, abilities, and other characteristics associated with current professional social work practice. The experience or education must be post-MSW degree. Experience and education satisfying this requirement must be active professional practice, which is paid/non-paid employment as a professional social worker, as defined by the appropriate state licensing board.
Quality of
Experience: Experience is only creditable if it is obtained following graduation with a Master's Degree in social work and if it includes work as a professional social worker directly related to the position to be filled. Qualifying experience must also be at a level comparable to social work experience at the next lower level.
GRADE REQUIREMENTS:
GS-09: This is the entry level grade for the GS-185 social work series and is used for social workers with less than one year of post-MSW experience and for social workers who are not yet licensed or certified at the independent practice level. Social workers at the GS-09 level are working toward completion of prerequisites for licensure or certification.
GS-11: Promotion to the GS-11 full performance level requires completion of a minimum of 1 year of post-MSW degree experience in the field of health care social work (VA or non-VA experience) and licensure or certification in a state at the independent practice level. OR, in addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
